Declaration of consent for participants of workshops and events
I acknowledge that this sports event carries with it the potential for injury. I hereby knowingly and freely assume all of the risks of participating in this event, both known and unknown, even if arising from the negligence of others, and assume full responsibility for my participation. I certify that I am physically fit and have not been advised otherwise by a qualified medical person.
In participating in this event, I hereby take action for myself and assign as follows: (A) Waive, release, and discharge from any and all liability for my personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my traveling to and from this event, the following entities or persons: the event organiser, Alexandre Medina Dos Santos (Griesplatz 26/14, 8020 Graz), and his employees, volunteers, members, and representatives, the event sponsors and event volunteers, (B) indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event.
Data Processing: I hereby agree to the processing and storage of my personal data (full name and E-mail-address) for the purpose of organising and implementing the Capoeira Workshop. Personal data are only disclosed to third parties if this is necessary for the implementation of the Capoeira Workshop. I have the right to withdraw my consent at any time via E-mail to senzalagraz@yahoo.com; however, I acknowledge that in this case I will not be able to participate in the Capoeira Workshop. Personal data are deleted after the event in consideration of the legally required minimum retention period.
E-mails: I consent to receiving E-mails in preparation of the event. I am aware that the E-mails I receive may contain personal data and I am aware of the associated risks, such as unauthorised access and processing by third parties.
Photos and videos: I understand that at this event or related activities I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders.
